Our agency is committed to providing clients and their families or representatives with clear information on how to access health records and file complaints regarding health record access.

How to Request Your Health Records

Consumers (clients) may request copies of their health care records directly from the Agency.
Please contact us by one of the following methods:

Requests will be handled in accordance with the Texas Health and Safety Code, Title 2, Subtitle I, Chapter 181, Subchapter C.
